Membership Cancellation request form

To terminate any contract, the member must notify Elite Core Studios by completing the Membership Cancellation Request form at least 30 days prior to any 3-month/ 6-month/ 12-month cycle expiration, otherwise the contract and membership will automatically renew every 3/6/12 months for another 3/6/12 month contract. 

Once emailed the cancellation, the studio will contact the member to set up a time for them to come into the studio to fill out the termination paperwork. Once the paperwork is signed and the final payment on the contract is received the contract will be terminated in 30 days.
