Membership Freeze Request Form

All 3/6/12 Month Membership Holders (excluding the 4 classes/month membership) are eligible for a "Freeze Time." The minimum freeze time is 1 month and the maximum freeze time is 3 months. There is a $50 hold fee for every month you freeze your account. To put your account on hold, you must fill out the Membership Freeze Request Form 7 days before the freeze to be eligible. Freeze dates are effective from the date after you fill out our Membership Freeze Request Form and cannot be back dated. Please make sure you fill out this form in advance!
